New Patients
If you are a new patient to our practice, please be sure to download, print and fill out our new patient packet prior to your arrival. Please be sure this information is filled out completely. If you do not have your paperwork completed, please arrive 30 minutes prior to your appointment time.

Your Medical Records
Should you need to have your records released from another physician, you will need to fill out a medical release form.

Should you request a copy of your medical records for insurance or personal use, applicable fees will apply.
Please allow 5-7 business days for any records to be copied, mailed or faxed from our office.
Urgent & Emergency Calls
If you are experiencing a life threatening situation, call 911 immediately or go to your closest ER. In most cases, Dr. Woods will be notified upon your arrival. If you are experiencing an urgent situation, go to your nearest urgent care center or ER and notify us the next business day to coordinate your follow up care.
Prescriptions
For prescription refills, please call 615-630-6969 at least 48 hours in advance of your anticipated need. This allows us the opportunity to examine your records ensuring your medication types and dosages are correct. When calling, have medication name, dosage level, frequency taken and pharmacy number available. Prescription refills will not be taken after hours. Prescriptions for antibiotics or narcotic pain medicines require an office visit.
